Fee-waiver review
We offer a fee-waiver program. To apply for a waiver of the $25 course fee, complete the Fee Waiver Request Form for final review and approval. We will review the information you provide and let you know whether you qualify for a fee waiver based on that information and whether your income is less than 150 percent of the poverty guidelines published by the United States Department of Health and Human Services. A chart is included with the form. You will be contacted within 1–2 business days with the results of your request.
Please submit both the completed fee-waiver form and either Schedule I (if prepared) or proof of income, such as one month of pay stubs, unemployment statements, or disability benefit statements, for review.
